*PLEASE NOTE: Both the #R002 and Code of Conduct Forms must be completely filled out.

*In addition to the aforementioned forms, we also need a copy/clear picture of your child’s birth certificate and a picture taken of your child from the shoulders and up. Please attach these files along with all other documents in the File Upload area of the Submission Form.

Instructions on Form Submission

1. Print and Scan: If you have the resources to do so (printer and scanner), you can print the documents, fill them out, then scan the documents and upload them to the “File Upload” field in the form below. This email will be sent to our registrar and you will then receive a Payment Form. If you have any questions or concerns regarding this process, please contact us.

2. Take a Photo With Your Mobile Phone: You can use your mobile phone to take an image of the printed and completed form. Just ensure that the entire document is in frame and crystal clear. You would then email the form to your computer (desktop, laptop, tablet) and the upload the form to the submission form below. If you have any questions or concerns regarding this process, please contact us.

3. Snail Mail: If you’d like to physically mail us the forms, you can do so by addressing the envelope to:

4. Using Photo Editing Software: If you are subscribed to Adobe Photoshop or have downloaded (Free to Use, click on the link ->) Gimp, you can upload the document to the photo editing software and, using the text tool, add the information. Once complete, save the document as a PDF and upload them to the “File Upload” field in the form below. This email will be sent to our registrar and you will then receive a Payment Form. UPLOAD DOCUMENT

From your computer, drag and drop the file into the software or select “File” -> “Open”, then select the directory where the file is located and import the file. You will presented with a prompt asking you which page to import. Select the option “Select All”. All pages will be highlighted in gray. Click “Import” from the lower right hand side of the prompt.

USING THE TEXT TOOL

From the image above, follow the enumerated steps:

  1. Select the Text Tool. This will be used in Step 5.
  2. Change the text size to 18px.
  3. Ensure the color is black. Also, ensure the “ab <- ->” value below the Justify Selection is 0.0.
  4. Ensure the font values in this box are 18px and the color is black.
  5. While the Text Tool (step 1) is still selected, drag the cursor in the desired field and type in the information. Proceed through the entire document using step 5.

LAYER ORDER

Ensure the Layer Order is as presented in the image above. To select between pages, click the “Eye” graphic under “Lock” to toggle visibility of the pages. It is important that the Text Layer is top-most in the layer order.

To add text to the second page, nest another Text Layer between Layers 1 and 2.

Once the entire document is filled to completion, i.e all pages completely filled, you’ll refer back to this area when ready to export. When ready to export, right click on the first Text Layer and select “Merge Down”. Do the same with the second Text Layer.

EXPORTING THE FILE I

Once the form is completed, you will need to export it back to the initial directory where the downloaded document was saved. Select “File” -> “Export as”.

Name the file whatever you’d like, but ensure to end the name with file extension “.pdf”. For instance, a proper name would present as:

BlackOaksRegistrationForm.pdf

Then click “Export” on the lower left hand side of prompt and export the file back to the initial directory. Last upload the file to the “File Upload” field in the form below.
